Home and Community Care Program for Younger People

The Home and Community Care for Younger People (HACC PYP) provides support to younger people (less than 65 years, or Aboriginal or Torres Strait Islander people aged less than 50 years) with day to day activities at home.

Services provided by West Wimmera Shire Council include:

  • Domestic Assistance
  • Personal Care
  • Delivered Meals and Centre-based Meals
  • Home Maintenance
  • Respite
  • Social Support Group
  • Individual Support

Who is eligible for services?

HACC PYP is targeted at people aged under 65 and Aboriginal people aged under 50 who need assistance with daily activities. Clients may require this assistance due to physical functional impairment, mental illness, chronic illness and short-term health needs.

Consumers are referred to a Living At Home Assessment Officer to discuss the right type level of support services.

Younger people who have an NDIS plan in place, are not eligible for HACC PYP.

How much will it cost me?

West Wimmera Shire Council’s HACC PYP services are heavily subsidised, and consumers are expected to contribute towards the cost of the services they receive, if they can afford to do so. These fees are determined by a self-declaration of income. Consumers will not be denied services if they are unable to contribute to the cost of the services.
